Fill out intake forms in advance and bring them to your first visit

These documents must be written, dated, and signed by the patient or by a person legally authorized to do so. If you have a printer at home then you can fill out this form on your computer and print it to bring to your first appointment. If you have trouble with the e-signature fields then you can sign them after you print the form. We recommend using Adobe Reader to fill out the forms from your computer. NEVER SEND COMPLETED MEDICAL FORMS OVER EMAIL.

* Any patient that has an outstanding balance of $200 that is over 60 days old or has more than $350 in outstanding balances at any given moment, will not be scheduled until the balance is paid.

Insurance

1. Medicare does not recognize Naturopaths and will not pay for any services from NDs. No CNM providers are participating with Medicare.
2. CNM in not participating with and will not submit any claims to any Health Net, Humana, or United Healthcare insurance policies. Patients with these insurance policies will be self-pay and payment is due at the time of each service. Self-pay patients are eligible to receive a 20% Time of Service discount for all services.

  • OHP Plans: CareOregon, Columbia Pacific, Jackson Care Connect, Yamhill Community Care, and Open Card/DMAP
  • Providence: Signature Network
  • Moda: Beacon, Connexus, Synergy
  • BCBS: Regence, HMA, Medicare Advantage
  • PacificSource
  • Aetna
  • Cigna

Patients are advised to verify coverage and benefits with their insurance company prior to scheduling. All of our providers are either Naturopaths or Acupuncturists, please make sure you have coverage for these provider types, based on the services you wish to receive. Claims are paid and processed based on your insurer’s discretion.

CNM is in the process of implementing Texting, Intake, and Telemedicine services with Yosi Health. This process is ongoing and implementation bugs are being worked out. Please contact us by phone or through your patient portal if you are experiencing any issues with this process so they can be promptly reported to and addressed by the new service provider. 
We recognize this has been a long and difficult process for everyone due to these added communication challenges and we thank you for your continued patience while we get everything in proper working order. We are all looking forward to getting back to smooth clinic operations soon!
